Whether you are a family member, carer, support coordinator, or healthcare professional, making a referral is quick, easy, and confidential.

We welcome referrals from:

Once we receive your referral, our team will:

  1. Review the information provided.
  2. Contact the participant or their representative to discuss needs and goals.
  3. Develop a personalised support plan that aligns with the participant’s aspirations and NDIS plan (where applicable).

This ensures a smooth transition into our services and the right level of support from the very beginning.
